What to Expect at Your Consultation

On your first visit you will be required to complete a registration form and medical history questionnaire.  To maximize our time together we ask that you arrive 10 minutes early to review our FAQ's and remove all necessary makeup. We will examine your skin and discuss skin care goals and treatment plans. Photos may be taken with permission.  Consultation fees are deducted from treatments booked if appointment occurs within 90days of initial consult.

Policies

Some Medical Aesthetic treatments are FDA approved for clients over 18yrs only.  Forever Flirty Lashes are safe for ages 13yrs and older. Any client under 18 must have parent permission to complete necessary forms.
Drivers License or Photo verification may be required.  

Payment

We are happy to accept Visa, Mastercard, or cash as payment.  Financing is available, please inquire.  
All treatments must be paid in full prior to service.  All Forever Flawless host credits have no expiration date.
All Consultation credits must be redeemed within 90 days of initial assessment to remain valid. 

Cancellations

Please contact us at least 24 hours in advance if you should need to cancel. We will attempt to contact you 48hrs before each appointment to review pre-appointment needs and confirm.   Failing to cancel with appropriate notice will result in the necessity to place 50% deposit for next appointment.

 Our premier partnership with Paradise Tanning provides another outlet for our clients to receive superior medical aesthetic treatments.  Conveniently located in Historic Olde Main Plaza, St. Albans. when you chose to visit us here  you can also enjoy the comfortable, clean, and modern tanning services provided.  As a Forever Flawless client you are eligible for Specials available only to our clients.  Parking is free and plentiful.  Our clients interested in scheduling appointments or party events outside of the home can arrange scheduling at this location.
